| Instructions: | Patient must Fed-Ex blood sample back to ELISA/ACT BIOTECHNOLOGIES in the test kit box using the proper Fed-Ex bag given to them by their Doctor, on the same day of the draw. Please call 800-553-5472 when filling out Fed-Ex form to make sure it is correct. If you have any questions about anything, please call. |
